Greetings Curtner Families,

We are planning for the next school year and would like to get a closer projection of our enrollment for the 2021-22 school year. The information we gather is critically important and we need your help.

Please indicate whether or not your child will be returning to Curtner next year.

• If you are not returning, please complete a Withdrawal Form (Please contact Ms. Nguyen-Truong @knguyentruong@musd.org)
• If you are unsure, please provide us with information you currently have in the comment section.
• Know that if you mark "unsure" it does not prevent you from keeping your seat at Curtner if circumstances change. We are using this form for estimating purposes only.
• Complete this form for each child if you have multiple children by May 7th.
